Summary
I think that we have tried the "Management is Accountable and Employees are Responsible" thing. How about make the employees the managers - They are accountable for the team's success. Hold the managers accountable to facilitate innovation and creativity in the team (using web 2.0?, decentralize etc.)

Managers can make this happen by setting meaningful goals and then helping remove obstacles for achieving the goal BUT letting employees define their objectives and plans for hitting those objectives.
